Back button doesn't function

Sometimes when I go to a page the back button turns gray
and does not function.  I have to access the history to
get back to a previous page.  Also, when closing a second
window it will sometimes close the first window.

Any ideas on how to correct this?

I have seen this a lot when a link inside one page is
clicked and another window opens.  The back button is
then grayed out.  I close that window and I am back at
the original one.  But I don't know why both windows
would close, the new and the original.
