Installing driver for unknown modem

I just reformatted a computer and am now reinstalling everything, and I
have no documentation or disks.

I don't know what make or model the modem (internal) is. How can I find
out?

The only thing on the actual modem is a  chip that says "LinkCom" and
one that says "Agere".

If the answer is to use Hayes drivers how do I know which one to use?
All I know is that it is a 56k modem.

There should be a FCC ID number printed on the board.

Go here: http://www.fcc.gov/oet/fccid/ to try and locate the manufacturer by FCC ID. This may get you started. Also if there is a Model number somewhere on the board you may try searching the internet for items relating to it.

Thanks, I figured out who made it and got drivers, but it doesn't seem
to work with the original computer, but it does with a different
computer...
