MRi 1456IVS ISA Modem?

Hi,

Does anyone have a manual for this modem? I'm trying to install it into a
Win98 system P11 233mhz, I've found the drivers, however the modem has 2
jumpers and 6 dip switches on board. I know the 2 jumpers set plug & play
on/off, if I set it to on, it seems to freeze the PC, I'm not sure about the
dip switches. If anyone has a manual, or knows where to get one (have tried
manufacture website) please can you email me

I generally determine the IRQ jumper functions by visual inspection:
