Why would a pop-up blocker block all pop-ups, then let
some pop-ups through? It work real good for like 3 months

Because the pop-up people figured out how to bypass some pop-up blockers.

It's the age old battle between offense and defense.

At some point in time you have to differentiate between valid pop-ups
and pop-ups caused by malware which has infected your PC.

The former are run by either the website itself, or the site-hosting
firm; these are advertisements and if they don't "pop up", the website
doesn't get revenue from the advertiser.  If you don't like the fact
that a website has ads on it, find a website that doesn't and/or email
the site administrator with your suggestions of other methods of keeping
the site open.

The latter are in the "adware" category and can usually be eliminated by
using ...
