I recently purchased a USB KVM switch and USB 4 port
HUB.  The KVM switch acts as a hub, funneling the
keyboard and mouse into one USB slot.  When I use either
device, I cannot get Windows to recognize either the
mouse or keyboard.  Any suggestions?

Try Removing all instances you can find for USB and mouse and keyboard in:

ControlPanel/System/DeviceManager (View by Type)

then reboot; Windows should reinstall your devices (or prompt you for
disks depending)
