Vista 64-Bit Custom Form for printer

I'm able to install a custom size form as I always have through the server
properties, but I'm not able to select the form through the device settings
for the printer that uses the custom form. It only lists all the normal forms
what windows normally has and not my custom form.

Does anyone know how to select your custom form for your printer? I've tried
to run the printer settings as an administrator and the custom form is still
not listed.

The custom form needs to be within the specification of the Custom_Size
portion of the PPD or GDP file with the driver.
