I get DW caused and invalid page fault in module kernel32.dll@0167:BFFco5ba....when I boot up and all icons are on desk top then I click the X and go on about my business any ideal ..can;t find this "DW" anywhere

I probably did "something" which resulted in the Accessories portion of the Program line in the startup to be removed. It now shows up on the Desktop. Can I correct? Also, I now get a seperate "logoff" line in the start menu, how can I remove? Thanks for ANY help.
